Position : Community Health Program Officer - Abuja

The Community Health Program Officer is responsible for designing, implementing, monitoring, and evaluating community-based health programs that advance public health goals. This role works closely with community partners, government agencies, healthcare providers, and internal teams to strengthen health systems, address health disparities, and improve health outcomes within target populations.
Key Responsibilities
Program Development & Implementation

  • Develop, plan, and manage community health initiatives in alignment with organizational strategy and public health priorities.
  • Conduct community needs assessments to identify gaps and opportunities for health interventions.
  • Lead the implementation of health promotion, disease prevention, and outreach programs.
  • Ensure program activities are evidence-based, culturally appropriate, and responsive to community needs.

Community Engagement & Partnership Building

  • Build and maintain strong relationships with community organizations, local leaders, healthcare institutions, and government agencies.
  • Facilitate community mobilization and participation in program activities.
  • Represent the organization at community meetings, coalitions, and external events.
  • Collaborate with stakeholders to co-design solutions that strengthen community health.

Monitoring, Evaluation & Reporting

  • Develop monitoring and evaluation (M&E) frameworks, tools, and indicators.
  • Track program performance, collect and analyze data, and prepare regular progress reports.
  • Use data to inform program adjustments and decision-making.
  • Ensure compliance with donor, regulatory, and internal reporting requirements.

Training & Capacity Building

  • Provide training, mentorship, and technical support to community health workers, volunteers, and partner organizations.
  • Develop educational materials and tools to support outreach and learning.
  • Strengthen community capacity for sustainable health initiatives.

Resource Management

  • Assist with program budgeting, forecasting, and stewardship of resources.
  • Support grant writing, donor reporting, and proposal development.
  • Coordinate procurement and logistics for program activities.

Qualifications
Education

  • Bachelor’s degree in Public Health, Community Health, Social Work, Nursing, or a related field is required.
  • Master’s degree in Public Health (MPH) or a related discipline is an asset.

Experience

  • 3–5 years of experience in community health, public health programming, or nonprofit program management.
  • Experience working with diverse communities and building partnerships with stakeholders.
  • Knowledge of public health principles, health promotion strategies, and community engagement approaches.

Skills & Competencies

  • Strong project management and organizational skills.
  • Excellent communication, facilitation, and interpersonal skills.
  • Ability to collect, analyze, and interpret data for decision-making.
  • Cultural competence and a strong commitment to health equity.
  • Ability to work independently and collaboratively in dynamic environments.
  • Proficiency in MS Office and familiarity with M&E tools or health information systems.

Work Environment

  • Fieldwork in community settings combined with office-based tasks.
  • Occasional evening or weekend work to support community activities and events.
  • Travel within project areas as required.
